Mục tiêu: Nghiên cứu đặc điểm hình ảnh siêu âm gan nhiễm mỡ theo phân độ của Hagen-Ansert ở cán bộ, nhân viên đến khám sức khỏe tại Bệnh viện Đại học Y dược Huế. Tìm hiểu một số yếu tố liên quan đến gan nhiễm mỡ. Phương pháp nghiên cứu: mô tả cắt ngang. Kết quả: Hình ảnh gan nhiễm mỡ trên siêu âm gặp ở độ tuổi 30-60 chiếm tỷ lệ 76,1%, gặp ở nam giới cao hơn nữ giới, tỉ lệ nam/ nữ = 1,23. Hình ảnh gan nhiễm mỡ hoàn toàn chiếm 96%, gan nhiễm mỡ khu trú chỉ chiếm 4%. Phân độ của Hagen-Ansert (1995) độ 1 hay gặp nhất chiếm 66,5%, độ 2 chiếm 30,5%, và độ 3 chiếm 3%. Tỉ lệ nam thừa cân có gan nhiễm mỡ trên siêu âm là 44,2%, nữ 29,3%; béo phì nam chỉ chiếm 1,2%, nữ chiếm 3,2%. Tăng Cholesterol, Triglycerid và LDL Cholesterol ở cả 3 phân độ gan nhiễm mỡ chỉ chiếm từ 26 - 32%. Có biểu hiện rối loạn lipid máu LDH chiếm 92,4%. Kết luận: Gan nhiễm mỡ độ 1 hay gặp chiếm đa số 66,5%, rối loạn lipid máu chiếm từ 26 – 32%. Không có sự thay đổi tương xứng giữa phân loại mức độ gan nhiễm mỡ trên siêu âm với nồng độ Triglicerid máu (p>0.05). Từ khóa: Gan nhiễm mỡ, siêu âm gan nhiễm mỡ. |
Objectives: This study describes the characteristics of hepatic steatosis on ultrasound imaging by grading of Hagen - Assert. Determining the factors related with fatty liver. Methodology: Cross-sectional study. Results: By studying 448 patients went to general examination at Hue University Hospital from 1/5/2013 to 1/9/2013, we have some of comments: 76.1% of patients has hepatic steatosis on ultrasound imaging in the aged of 30-60 years, common in men than women,with male – female ratio:1.23. The majority (96%) hepatic steatosis was complete all lobes, and the remaining 4% patient was focal fatty liver. Grading hepatic steatosis by Hagen-Ansert classification (1995): there were 66.5% grade 1, 30.5% grade 2, and grade 3 of 3.0%. 26 - 32% patients’ hepatic steatosis on ultrasound combinated with increasing of Lipidemia (Cholesterol, Triglycerid and LDL Cholesterol). Conclusion: There were 66.5% patients grading level 1 by Hagen-Ansert classification for hepatic steatosis. Increasing of Lipidemia index represented only 26 – 32% of patients. Our study did not found significant compatible change between the hepatic steatosis on ultrasound imaging and Triglycerid blood concentrations (p > 0.05).
