Mục tiêu: (i) Mô tả các đặc điểm lâm sàng, cận lâm sàng của bệnh xuất huyết não - màng não ở trẻ em; (ii) Xác định các yếu tố nguy cơ, nguyên nhân gây xuất huyết não ở trẻ em. Phương pháp nghiên cứu: Nghiên cứu mô tả cắt ngang. Kết quả: Qua nghiên cứu 34 trẻ xuất huyết não- màng não, ghi nhận trẻ trai mắc bệnh nhiều hơn trẻ gái. Tỷ lệ nam/nữ: 2,4/1. Sự khác biệt có ý nghĩa thống kê ở nhóm trẻ dưới 1 tuổi p<0,05; 76,5% trẻ nhập viện trong 24 giờ đầu khởi phát bệnh; Ở trẻ dưới 1 tuổi, triệu chứng khởi phát gặp nhiều nhất là co giật 45,8%, bú kém 25%. Ở trẻ từ một tuổi trở lên, đau đầu là triệu chứng xuất hiện đầu tiên 50%, thay đổi ý thức, co giật, yếu chi không thường là triệu chứng khởi phát. Trẻ nằm viện trên 7 ngày chiếm tỷ lệ cao nhất 50%. Tỷ prothrombin từ 20 – 50% chiếm tỷ lệ cao nhất 45,5%. Siêu âm thóp ở trẻ còn thóp, xuất huyết não – màng não độ II chiếm tỷ lệ cao nhất 45,5%. Hình ảnh xuất huyết não trên CTsan sọ não, xuất huyết nhu mô chiếm tỷ lệ cao nhất 75%. Trẻ nhẹ cân, ngạt sau sinh hoăc bệnh tim bẩm sinh là yếu tố nguy cơ cao gây xuất huyết não. Nguyên nhân hay gặp xuất huyết não-màng não là xuất huyết giảm tỷ chiếm tỷ lệ cao nhất 64,7%, tuy nhiên nguyên nhân do dị dạng động tĩnh mạch não cũng chiếm tỷ lệ khá cao 17,6%. Kết luận: Trẻ dưới 1 tuổi triệu chứng khởi phát co giật, bú kém; trẻ từ một tuổi trở lên thay đổi ý thức là triệu chứng xuất hiện đầu tiên. Nguyên nhân gây xuất huyết não hay gặp do giảm tỷ prothombin, tiếp đến là dị dạng động tĩnh mạch não. Từ khóa: Xuất huyết não-màng não |
Objective: (i) Describe the clinical features, the subclinical intracranial hemorrhage in children. (ii) Identify the risk factors, causes of intracranial hemorrhage in children. Methods: Cross-sectional escriptive study. Results: Study on 34 children with intracranial hemorrhage, study reveals that more boys than girls. The rate of male / female: 2.4/1. The difference was statistically significant in infants under 1 year of age, p < 0.05. 76.5% of children hospitalized in the first 24 hours of onset. In children under 1 year of age, symptom onset is the most common seizure 45.8 %, poor feeding 25%. In children aged older one year-old, headaches are symptoms first appear 50%, changes in consciousness, seizures, weakness, usually no onset symptoms. Children over 7 days of hospitalization accounted for the highest 50 %. The prothrombin rate from 20-50% accounted for the highest percentage of 45.5%. The fontanelle ultrasound in infants, highest percentage of cerebral hemorrhage - grade II 45.5%. With CTsan brain, parenchymal hemorrhage accounted for the highest percentage of 75%. Low- birth weight, asphyxia birth, congenital heart disease highest risk factor caused an intracranial hemorrhage. Common causes bleeding in the intracranial haemorrhage reduce the highest percentage of 64.7%, however, caused by cerebral arteriovenous malformations also relatively high proportion of 17.6%. Conclusions: Children under 1 year- old onset seizures, poor feeding, and older children with a consciousness change. Common causes of incranial hemorrhage by reducing prothombin rate, cerebral arteriovenous malformations. Key words: intracranial hemorrhage |
